You may contact your insurance company, make a request for mental health or behavioral health services, and ask about coverage for an out-of-network provider. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- What are my out-of-network benefits?
Clients who pay out-of-pocket will be provided a statement at the end of each month which may be submitted to their insurer, benefits department for reimbursement via a health savings account, or for income tax purposes.
FEE SCHEDULE
- $225 Initial Session
- $165 Follow-Up
- $200 per hour for report writing (charged by the quarter hour)
Cash, Check, Bank, or Major Credit Cards are accepted. Additional fees may apply for medical and legal consultation and will be charged by the quarter hour. ;
CANCELLATION POLICY
48 Business Hours notice is required in advance of cancelled appointments or you will be charged the price of a full session. Exceptions may be made at our discretion.
